BIBW2992 mostra una potente attività contro le forme sia wild-type che mutanti di EGFR e HER2. È simile al Gefitinib in potenza contro l'EGFR L858R, ma circa 100 volte più attivo contro il doppio mutante L858R-T790M EGFR resistente al Gefitinib. Questo composto esibisce potenti effetti sulla fosforilazione di EGFR e HER2 in vivo. Si confronta favorevolmente con i composti di riferimento (come Lapatinib et al.) in tutti i tipi di cellule testati, come la linea cellulare di carcinoma epidermoide umano A431 che esprime EGFR wt, le cellule murine NIH-3T3 trasfettate con HER2 wt, così come la linea cellulare di cancro al seno BT-474 e la linea cellulare di cancro gastrico NCI-N87, che esprimono HER2 endogeno.

Il dominio Protein Tyrosine Kinase wild type dell'EGFR umano, così come quello del doppio mutante EGFR L858R/T790M, sono fusi alla glutatione-S-transferasi (GST) ed estratti. L'attività enzimatica viene quindi saggiata in presenza dell'inibitore BIBW2992, diluito in serie in DMSO al 50%. Un polimero casuale pEY (4:1) viene utilizzato come substrato e il pEY biotinilato (bio-pEY) viene aggiunto come substrato tracciante. Il dominio Protein Tyrosine Kinase di HER2 viene clonato utilizzando il sistema del baculovirus ed estratto in modo simile a quello della Protein Tyrosine Kinase EGFR. La somministrazione orale giornaliera di BIBW2992 a 20 mg/kg per 25 giorni determina una regressione tumorale drammatica con un rapporto cumulativo volume tumorale trattato/controllo (rapporto T/C) del 2%. La ridotta fosforilazione di EGFR e AKT è confermata mediante colorazione immunoistochimica delle sezioni tissutali. Pertanto, come lapatinib e neratinib, questo composto è un inibitore della Protein Tyrosine Kinase (TKI) di nuova generazione che inibisce irreversibilmente le Protein Tyrosine Kinasi del recettore 2 del fattore di crescita epidermico umano (Her2) e del recettore del fattore di crescita epidermico (EGFR). Non è solo attivo contro le mutazioni di EGFR bersagliate dai TKI di prima generazione come Erlotinib o Gefitinib, ma anche contro quelle insensibili a queste terapie standard.
