La Melatonin interagisce con il radicale idrossilico altamente tossico con una costante di velocità equivalente a quella di altri spazzini di radicali idrossilici altamente efficienti. Secondo quanto riferito, neutralizza il perossido di idrogeno, l'ossigeno singoletto, l'anione perossinitrito, l'ossido nitrico e l'acido ipocloroso.

Si ritiene che questo composto elimini il radicale idrossilico altamente tossico, l'anione perossinitrito e, possibilmente, il radicale perossile. Secondo quanto riferito, elimina il radicale anione superossido e spegne l'ossigeno singoletto. La sostanza stimola i livelli di mRNA per la superossido dismutasi e le attività della glutatione perossidasi, della glutatione reduttasi e della glucosio-6-fosfato deidrogenasi (tutti enzimi antiossidanti), aumentando così la sua capacità antiossidante.

In sistemi acellulari, questa sostanza chimica ha dimostrato di eliminare direttamente H2O2, l'ossigeno singoletto (1O2) e l'ossido nitrico (NO*), con scarsa o nessuna capacità di eliminare il radicale anione superossido (O2*-) in vitro. Disintossica anche direttamente l'anione perossinitrito (ONOO-) e/o l'acido perossinitroso (ONOOH), o la forma attivata di questa molecola, ONOOH*. La Melatonin agisce come un diretto spazzino di radicali liberi con la capacità di disintossicare sia le specie reattive dell'ossigeno che quelle dell'azoto.

Inibisce l'accumulo di cAMP nella maggior parte delle cellule esaminate, ma gli effetti dell'indolo su altri messaggeri sono stati spesso osservati solo in un tipo di cellule o tessuti, finora. Questo composto regola anche i fattori di trascrizione, vale a dire, la fosforilazione della proteina legante l'elemento di risposta al cAMP e l'espressione di c-Fos.
