Triptolide è un triepossido diterpenico con potenti proprietà immunosoppressive e antinfiammatorie. Questo composto ha dimostrato di inibire l'espressione di IL-2 nelle cellule T attivate a livello del fattore purine-box/nucleare e dell'attivazione della trascrizione mediata da NF-κB. Inibisce la proliferazione e la formazione di colonie di cellule tumorali a concentrazioni estremamente basse (2–10 ng/mL). Questa sostanza chimica ha un'attività inibitoria sulle cellule di cancro al seno, allo stomaco e sulla linea cellulare di leucemia HL-60. Induce l'apoptosi nelle cellule tumorali bloccando l'attivazione di NF-κB e sensibilizzando le cellule tumorali alla morte cellulare programmata indotta da TNF-α.

Triptolide sinergizza con la ciclosporina A nel promuovere la sopravvivenza del trapianto in modelli animali e nella soppressione della malattia del trapianto contro l'ospite nei trapianti allogenici di midollo osseo. Inoltre, induce l'apoptosi nelle cellule tumorali e potenzia l'induzione di apoptosi da parte del fattore di necrosi tumorale (TNF-α) in parte attraverso la soppressione dell'induzione di c-IAP2 e c-IAP1. Il trattamento con questo composto per 2-3 settimane inibisce la crescita di xenotrapianti formati da quattro diverse linee cellulari tumorali (melanoma B16, cancro al seno MDA-435, cancro alla vescica TSU e carcinoma gastrico MGC80-3), indicando che TPL ha un ampio spettro di attività contro i tumori che contengono sia forme wild-type che mutanti di p53. Inoltre, inibisce la metastasi sperimentale delle cellule B16F10 ai polmoni e alle milze dei topi. Ha attività in vitro e in vivo contro modelli murini di malattia del rene policistico. LD50: Topi 0,83 mg/kg (i.v.).
