Idelalisib (CAL-101) is a selective p110δ inhibitor with IC50 of 2.5 nM in cell-free assays; shown to have 40- to 300-fold greater selectivity for p110δ than p110α/β/γ, and 400- to 4000-fold more selectivity to p110δ than C2β, hVPS34, DNA-PK and mTOR. Idelalisib also stimulates autophagy.

Idelalisib (CAL-101) non è sensibile ad altre subunità di PI3K di classe I, incluse p110α, p110β e p110γ. Blocca specificamente l'espressione di CD63 mediata da FcϵR1 p110δ con una EC50 di 8 nM nei basofili primari. Questo composto mostra una maggiore attività nelle cellule di leucemia linfoblastica acuta a cellule B (B-ALL) e leucemia linfatica cronica (CLL) rispetto alle cellule di leucemia mieloide acuta (AML) e neoplasia mieloproliferativa (MPN). Produce la riduzione di pAktS473, pAktT308 e del bersaglio a valle S6 nelle cellule SU-DHL-5, KARPAS-422 e CCRF-SB con una EC50 da 0,1 a 1,0 μM.

Induce citotossicità selettiva nelle cellule di CLL indipendentemente dallo stato mutazionale di IgVH o dalla citogenetica interfase, principalmente attraverso un meccanismo dipendente dalle caspasi. Il composto induce citotossicità preferenzialmente nelle cellule di CLL rispetto ai linfociti B normali, senza produrre citotossicità in altre cellule ematopoietiche, rispetto a LY294002. Manca di potenziale citotossico diretto nei confronti delle cellule T e delle cellule natural killer (NK). Può inibire la produzione di citochine infiammatorie, come IL-6, IL-10, TNF-α e IFN-γ, e citochine indotte dall'attivazione, come CD40L. Antagonizza anche la sopravvivenza delle cellule di CLL mediata da CD40L.

Induce un accumulo di cellule in G1 e una diminuzione della popolazione in fase S nelle cellule L1236 e L591, il che indica questo composto come una nuova strategia per il trattamento del linfoma di Hodgkin (HL).

Saggio PI3K
Il saggio PI3K viene eseguito su lisati di cellule intere da cellule CLL o linfociti B normali. Viene eseguito un saggio ELISA PI3K. In breve, gli estratti di cellule intere vengono aggiunti a una miscela di substrato PI(4,5)P2 e tampone di reazione contenente adenosina trifosfato (ATP) e lasciati incubare a temperatura ambiente. La reazione viene interrotta aggiungendo il rivelatore PI(3,4,5)P3 miscelato con EDTA (acido etilendiamminotetraacetico) e lasciato incubare a temperatura ambiente per 1 ora. Trascorso questo tempo, la miscela viene trasferita da ciascun pozzetto a una piastra ELISA PI3K e lasciata incubare per 1 ora. Le piastre vengono lavate e quindi incubate con un rivelatore secondario per 30 minuti. Le piastre vengono lavate di nuovo e viene aggiunta una soluzione di 3,3′,5,5′-tetrametilbenzidina per 5 minuti, dopodiché viene aggiunto H2SO4 per interrompere tutte le reazioni. Le piastre vengono lette a 450 nm su un lettore di piastre a 96 pozzetti Labsystems.
